Applications for passports are available from the City Clerk’s Office on the 2nd Floor of Star City Hall at 10769 West State Street, Star Idaho 83669.

We will accept passports M-F 9:00am-4:00pm via appointment only.

You can also go to https://travel.state.gov/content/travel/en/passports/requirements/forms.html and print out the needed passport application. This site also contains information on passport services and other passport-related information.  PRINT ALL PAGES SINGLE SIDED!

To obtain a passport, you must come into the Clerk’s Office in person. You will need:

  1. Completed, but unsigned, passport application (DS-11). It must be signed in the presence of an authorized Passport Acceptance Agent in the City Clerk’s office.  
  2. Two identical 2”x2” passport photos. These must have been taken in the last six months and have a light background.  (Star City Hall does offer passport photo services for an additional charge of $25.00 - Payable to the City of Star in addition to all other fees)
  3. Birth certificate with a raised seal, issued by the state where applicant was born. Hospital certificates and birth abstracts are not acceptable.
    If you are a naturalized citizen, you must bring your original naturalization papers. All original documents that are sent with your application are returned to you with your passport.
  4. Current driver’s license or some other form of photo ID.

The US Department of State does not allow us to accept a Counter Check.  Please only bring Cashier's Check, Money Order or regular Bank Draft Check.

FEES for routine passport applications may be paid using a money order, cashier’s check or personal check. Fees are non-refundable.

PASSPORT BOOK
For ages 16 and older: $130 Payable to Dept. of State, plus
$35 Payable to City of Star

For ages under 16: $100 Payable to Dept. of State, plus
$35 Payable to City of Star

PASSPORT CARD
For ages 16 and older: $30 Payable to Dept. of State, plus
$35 Payable to City of Star

For ages under 16: $15 Payable to Dept. of State, plus
$35 Payable to City of Star

Passport Renewal (Form DS-82)
If you can answer “yes” to all the following statements, you can use the DS-82. (Star City Hall Clerk’s Office does not do renewals but carries the DS-82 forms).

  1. I can submit my most recent U.S. passport book and/or U.S. passport card with this application.
  2. I was at least 16 years old when my most recent U.S. passport book and/or U.S. Passport card was issued.
  3. I was issued my most recent U.S. passport book and/or U.S. Passport card less than 15 years ago.
  4. I use the same name as on my most recent U.S. passport book and/or U.S. Passport card

-OR-

  1. I have had my name changed by marriage or court order and can submit proper documentation to reflect my name change.

If you answered NO to any of the four statements above, you cannot use the DS-82. You must apply on application form DS-11 by making a personal appearance before an acceptance agent authorized to accept passport applications, such as the Clerk’s Office at Star City Hall.

The non-refundable total fee for the renewal is $130.

Expedited Service
For any expedited service (e.g., first-time application, renewal, additional pages, name change) there is an additional service fee of $60.

We strongly suggest that you use overnight delivery service to return your newly issued Passport Book to you. Overnight delivery service is not available for passport cards, which are delivered via First Class Mail only. The current Overnight Delivery Fee is $19.53.
