Passport Books

The City of Star accepts passport applications BY APPOINTMENT ONLY.  Please read this page before scheduling your appointment using the link at the bottom of this page.

Appointments are available Monday – Friday 8:30 am-3:30 pm. Our schedule goes out 45 days.  Each day the 45th day opens up.  Cancelations will show up on the schedule but are taken quickly. 

If you no longer need your appointment, please cancel using the link you received when you made your appointment or by calling our office.  Canceling an appointment you will not use allows someone who needs it the opportunity to schedule that time.

Review the requirements below and check the federal passport website at travel.state.gov to determine any special requirements BEFORE scheduling your appointment. 

To be prepared and to streamline the process please have ALL FORMS FILLED OUT and documents ready and available before arriving at your appointment.  You may contact our office at 208-286-7247 ext 9010 or passports@staridaho.org with any questions. 

Applications can be obtained and printed at https://travel.state.gov/content/travel/en/passports/how-apply/forms.html.  PRINT ALL PAGES SINGLE SIDED.  Applications are also available in the Clerk’s Office on the 2nd floor of City Hall, 10769 W State Street, Star Idaho. 

SCHEDULE YOUR PASSPORT APPOINTMENT

To obtain a passport, you must come to the Clerk’s Office in person.  You will need:

  • Completed, but unsigned, passport application (DS-11).  It must be signed in front of an authorized Passport Acceptance Agent in the City Clerk’s office. 
  • Current Driver’s License, State-Issued Identification Card, or proof of citizenship.  OUT OF STATE IDENTIFICATION REQUIRES SECONDARY IDENTIFICATION.
  • Original Birth Certificate with a raised seal, issued by the state where the applicant was born, State Certified copy of Birth Certificate, or original passport.  Hospital Certificates and birth abstracts are not acceptable.
  • Original Naturalization papers, if you are a naturalized citizen.
  • Two identical 2”x2” passport photos.  These must have been taken in the last six months and have a light background.  (photos are the #1 reason for a rejection) Star City Hall does off passport photo services for an additional charge of $25.00.  - Payable to the City of Star in addition to all other fees.

All original documents that are sent with our application are returned to you by mail.

FEES

Department of State fees must be paid with PERSONAL CHECK, CASHIER’S CHECK OR MONEY ORDER.  NEW (07/23/2024) - Please bring a separate check for each application. 

Fees paid to the City of Star can be paid with PERSONAL CHECK, CASHIER’S CHECK, MONEY ORDER, CREDIT CARD or CASH. 

All fees are non-refundable.

PASSPORT BOOK

For ages 16 and older: $130 payable to Dept. Of State, plus $35 payable to City of Star

For ages under 16: $100 payable to Dept. Of State, plus $35 payable to City of Star

PASSPORT CARD

For ages 16 and older: $30 payable to Dept of State, plus $35 payable to City of Star

For ages under 16: $15 payable to Dept of State, plus $35 payable to City of Star

If you are getting a book and a card at the same time, there is only one fee to the City of Star per applicant.

EXPEDITED SERVICE

For any expedited service there is an additional fee of $60 payable to the Dept. Of State.

If you are in a hurry for your passport, overnight delivery service is available to return your Passport Book to you.  Overnight delivery service is not available for Passport Cards, which are delivered via First Class Mail Only.  The current Overnight Delivery Fee is $21.36.

RENEWALS

Renewing your passport?  See travel.state.gov website for full requirements and instructions for renewals. 

If you can answer “yes” to all the following statements, you can use the DS-82 Renewal Form.  Star City Hall does not handle renewals but carries the DS-82 forms.  Adult renewals can also be completed online at https://travel.state.gov/content/travel/en/passports/have-passport/renew-online.html.

  1. I can submit my most recent U.S. passport book and/or U.S. passport card with this application.
  2. I was at least 16 years old when my most recent U.S. passport book and/or U.S. passport card was issued.
  3. I was issued my most recent U.S. passport book and/or U.S. passport card less than 15 years ago.
  4. I used the same name as on my most recent U.S. passport book and/or U.S. passport card.

-or-

I have had my name changed by marriage or court order and can submit proper documentation to reflect my name change.

If you have answered NO to any of the statements above, you cannot use the DS-82, and you must apply for a passport using application form DS-11 by making a personal appearance before an Acceptance Agent in the Clerk’s office. 

The non-refundable total fee for the renewal book is $130.
